Dr Gareth September is a qualified plastic surgeon who specialises in microvascular free tissue transfers, lower limb reconstruction and cosmetic surgery. Dr September administers Botox and fillers to enhance and contour facial features. Lipofilling, liposuction, thigh, breast, face, neck and thread lifts, as well as otoplasty and large lip correction, are the doctor's surgical specialities.

Dr September graduated with an MBChB from the University of Stellenbosch and is a member of The College of Plastic Surgeons of South Africa. The doctor is a member of the Association of Plastic, Reconstructive and Aesthetic Surgeons of South Africa (APRASSA). Dr September runs his practice from Life Pallotti Hospital in Pinelands, Cape Town.

  • Large lip correction
    Some people who are unhappy with the size of their lips undergo large lip corrective surgery. Also, a patient who is unsatisfied with a previous lip job opts for corrective lip surgery. To diminish the size of the lips, the doctor removes excess tissue. After the lip procedure, the patient may notice slight swelling and pain, but Dr September prescribes medication to relieve this discomfort.
  • Facelift/neck lift/brow lift/thread lift
    For a face, neck and thread lift, the surgeon lifts and tightens the skin. A neck lift may involve liposuction to remove a surplus of fat. A brow lift may be done openly or endoscopically.   • Lipofilling
    Lipofilling is a fat transfer procedure. Lipofilling is mainly done to plump the lips, cheeks and breasts. Unlike fillers that require regular touch-ups after every few months, Lipofilling is a procedure with long-term results. The patient’s fat from the tummy and thighs is used to plump areas of the body. Through liposuction, Dr September suctions fat cells from the thighs or abdomen and sterilises and preps the fluid. Once the surgeon disinfects the fat solution, he injects it into areas of the body that require enhancement.
  • Otoplasty
    An otoplasty is corrective ear surgery. Otoplasty is a surgery that the plastic surgeon performs to alter the shape, size and placement of the ears. The technique that the surgeon utilises depends on the patient’s needs. Dr September makes incisions in the folds of the ear or behind it. As soon as a surplus of skin and cartilage has been removed, the surgeon aligns the cartilage in its new position and uses internal stitches to hold it in place.
  • Breast lift
    A breast lift or mastopexy is a breast rejuvenation procedure. Dr September lifts and contours the breasts for a revitalised appearance. Although a breast lift doesn’t significantly increase the size of the breasts, it does restore the shape of the breasts. Also, a surgeon may perform a breast lift in conjunction with breast enhancement surgery to increase the size of the breasts. The plastic surgeon carves near the areolas around the creases of the breast and removes extra skin. The nipples are raised and the breasts, reshaped.

    Lipofilling is a fat transfer technique that the plastic surgeon uses to enhance the size of the breasts. Lipofilling a popular procedure as it involves minimal scarring and has permanent results. The fat that’s used to plump the breast is 100% natural, and there's minimal risk of infection. Fat is retrieved from the tummy, buttocks or thighs. A specialised spin system separates healthy fat cells from oily, non-viable cells. After the disinfection and preparation of fat cells, the surgeon injects healthy fat into the breasts.
  • Liposuction
    Liposuction and a medial thigh lift are cosmetic surgical procedures that Dr September performs to contour thighs. Liposuction is a favourable cosmetic option that the surgeon carries out to remove excess fat and shape other regions of the body, particularly the breasts and thighs. Before the surgeon uses a vacuum to suck out fat cells from the body, he injects a saline fluid into the surgical site. Tumescent liposuction is the most widespread type of liposuction that is used today. Even though all the fat cells are removed, patients need to follow a healthy diet and exercise regime.
  • Medial thigh lifts
    Dr September performs a medial thigh lift to tighten the skin in the upper thigh region. A medial thigh lift tightens loose skin and reduces signs of ageing. Patients who have recently lost weight are ideal candidates for this procedure. The surgeon administers a general anaesthetic and cuts from the groin to the buttocks. Once the skin is raised, extra tissue and fat are carefully dissected. There’s minimal discomfort after the procedure.
  • Fillers
    Juvederm, Voluma and Volbella fillers are injectable hyaluronic acid. Fillers mimic natural fat and are used to fill in sunken cheeks, lips and hands. Soft fillers don’t last as long as fillers used in less mobile regions like the cheeks. A patient who has lost volume in his or her lips and cheeks is an ideal candidate for fillers. Injectable hyaluronic acid softens folds, wrinkles and lines. Lip fillers last for six months, and cheek fillers last for about eighteen months.
  • Botox
    Botox is a painless non-invasive procedure. Botox or Botulinum toxin deactivates muscles and reduces signs of ageing. Dr September gently inserts tiny needles around the eyes and forehead to remove lines and crow’s feet. Botox softens overactive muscles and rejuvenates the face. Ten days after Botox treatments, the patient starts to notice results. Botox lasts for up to four months. A touch-up is needed when facial muscles kick into overdrive, and lines begin to reappear.
  • Microsurgery
    Dr September performs microsurgery to reconstruct the breast, bone, neck and head. Not only orthopaedic surgeons utilise this technique, but, plastic surgeons also use it. Reconstructive microsurgery is the transferal of fat, tissue, skin, bone and blood vessels to wound beds. Facial re-animation is an advanced reconstructive procedure that Dr September performs to treat facial paralysis. Gracilis muscle transfer is a procedure that the plastic surgeon performs to restore facial symmetry and one’s smile. A portion of the Gracilis muscle, that’s found in the upper thigh, is transferred to the face to restore facial expression.
  • Head and neck surgery
    Head and neck surgery involves reconstruction of the neck through the use of muscle, skin, blood vessels, tissue and bone. The plastic surgeon performs microsurgery to repair the damage after he removes tumours in the neck and head. A free flap procedure involves the use of tissue with a healthy blood supply which the surgeon takes from a donor site and transfers it to the wound bed. Dr September connects flap blood vessels to the blood vessels in the neck.
  • Breast reconstruction
    A woman who undergoes a mastectomy to treat breast cancer requires breast reconstructive surgery. There are several breast reconstructive procedures available. Breast implants, flap procedures and nipple and areola reconstruction are reconstructive breast techniques. Patients can choose saline or silicone, “gummy bear” implants to reconstruct the breast. A tissue transfer is also implemented to secure breast implants in place. The tissue is usually extracted from another area of the body that could be the back or stomach. Flap procedures are more complex than the implantation of breast implants. Dr September specialises in Latissimus dorsi flap. Latissimus dorsi flap is a procedure where tissue is retrieved from the upper back.
  • Lower limb
    Trauma to the lower extremity may occur due to injuries, falls and motor vehicle accidents Reconstructive lower limb surgery may require localised flaps, skin grafts and if necessary, microsurgery. In the case of a defect or open wound, the surgeon performs a skin graft and takes skin that doesn’t have a blood supply to camouflage the affected area. When soft tissue has been damaged, the surgeon transfers tissue flaps that comprise of muscle to the affected regions. If blood vessels are damaged, the surgeon will transfer free flaps that consist of fat, skin, muscle and bone to the affected areas.
  • Hand surgery
    Hand surgery is needed to repair hand injuries, restore mobility and reduce pain. Carpal tunnel syndrome, rheumatoid arthritis and Dupuytren's contracture require corrective hand surgery. Rheumatoid arthritis is a condition that affects joints. Rheumatoid arthritis of the hand affects the movement of the wrist and fingers. Carpal tunnel syndrome is an uncomfortable condition that affects the hands and wrists. Patients who suffer from carpal tunnel syndrome experience numb fingers and a pins and needles sensation. Carpal tunnel release surgery alleviates pressure on the median nerve and restores wrist and hand mobility. Dupuytren's contracture occurs when a web of tissue extends from the palm to the fingers. A partial fasciectomy is a corrective procedure that Dr September performs to remove tangles of tissue and straighten the fingers.
  • Skin cancer surgery
    Skin cancer occurs when skin cells multiple and eventually snowball. Basal cell, squamous cell carcinoma and melanoma are different forms of skin cancer. Melanoma is a dangerous form of skin cancer that starts in skin cells called melanocytes. Melanocytes produce the pigment, melanin which colours the skin. Melanoma may arise from moles or occur on its own. One follows the ABCDEs (asymmetry, border, colour, diameter, evolution) to diagnose melanoma. Basal cell carcinoma is a progressive skin cancer that can be easily treated if it's caught early enough. Dr September performs an excision to remove the lesion along with a margin of healthy tissue.
